My pick is Pro T Flare D.
It's almost like R and S Flare C but it's from the T formation. You've got 2 safety valves, both running backs. A Tight End who sits down in the middle and WR1 and WR2 once again go deep and are separated enough that if they're both open and the man defender commits to one, he'll have hard time getting to the other guy if the ball it thrown the other WR.

This is the best pass play in the game IMO.

R and S Z Fly is pretty good too.
The only problem I have with that is only 4 receivers run a route instead of 5 receivers.
